Assumption of the Risk and Waiver of Liability Relating to Injury and COVID-19
I do hereby assume full responsibility for any and all damages, injuries, and losses that I may sustain or incur in any way while attending and participating at Albany Aikido. I hereby waive all claims against the instructors, students, and operators of the Dojo for any claims or injuries that I may sustain.
Albany Aikido (“the Dojo”) has put in place preventative measures to reduce the spread of COVID-19; however, the Dojo cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending the Dojo could increase your risk and your child(ren)’s risk of contracting COVID-19.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I and/or my child(ren) may be exposed to or infected by COVID-19 by attending the Dojo. I understand that the risk of becoming exposed to or infected by COVID-19 at the Dojo may result from the close interactions between practitioners of Aikido.
I hereby grant Albany Aikido permission to use my/my child’s likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration.
I further understand that the terms of this agreement are legally binding and certify that I am signing this agreement, after having carefully read it, of my own free will.
